United Heritage Life Insurance - Actives

July 1, 2008 - July 1, 2009

Information provided is in summary format.  Any difference between the summary provided and actual contract will be settled in favor of the contract.

Eligibility  

You are generally eligible for Life insurance coverage if you are an active full-time employee working a minimum of 20 hours per week.  

Coverage Amounts  

$40,000 

Life Reduction Benefit

Life insurance amounts will reduce to $20,000 at age 70.

Accidental Death and Dismemberment  

If you sustain an accidental Bodily Injury which results in loss of life, sight, or limb within 365 days of the injury you are entitled to certain additional benefits.

Accelerated Death Benefit

If you become terminally ill and are not expected to live more than 12 months, you may request up to 50% of your life insurance amount up to $20,000.  A doctor must certify your condition in order to qualify for this benefit.  The death benefit will be reduced by any benefits paid under this option.  

Seat Belt Benefit  

An additional Accidental Death Benefit of 10% will be paid if you die as a result of an automobile accident and you were wearing a seatbelt. 

Conversion Privilege

If your insurance terminates because your employment terminates, or certain other reasons you may be entitled to convert all or part of your Group Life Insurance to an Individual Life Policy without submitting evidence of insurability.  

Waiver of Premium  

If you are totally disabled and are less than 60 years of age at the time disability begins, the Life Insurance carrier will continue your coverage with no premiums due, provided that the disability is reported within 12 months of its start and you submit any required proof of disability, and meet the definition of total disability.  

Coverage for Dependents

Spouse; $6,000

-2 weeks of age to less than 6 months of age; $500

-6 months of age to 19, or to age 23 if they meet dependent eligibility; $6,000
