United Heritage Short Term Disability Benefits

July 1, 2008 - June 30, 2009

Information provided is in summary format.  Any difference between the summary provided and actual contract will be settled in favor of the contract.

Eligibility You are generally eligible for LTD coverage if you are an active full-time employee working a minimum of 20 hours per week.
Weekly Benefit 60% of basic weekly earnings to a maximum of $1,000 per week.
Elimination Period Accident Benefits begin on day 8
Sickness Benefits begin on day 8
Maximum Benefit Duration 12 weeks
