United Heritage Supplemental Life Insurance

Eligibility  

You are generally eligible for Life insurance coverage if you are an active full-time employee working a minimum of 20 hours per week.  
Coverage Amounts  
     Member $10,000 increments not to exceed 5 times your Basic Annual Earnings up to $500,000 maximum
     Spouse $6,000
     Child Over 14 days old less than 6 months old:  $500
6 months old or older:  $6,000
