United Heritage Life Insurance

Eligibility  

Class 1:  All active, full-time employees working 20 hours or more per week for five or more consecutive months

Class 2:  All retired administrators

Class 3:  All retired employees, excluding administrators

Class 4:  All active, full-time administrators

Coverage Amount  

Class 1:  $50,000 ($7.25/month, employer-paid)

Class 2:  $11,000 ($16.50/month, retiree-paid)

Class 3:  $5,500 ($8.25/month, retiree-paid)

Class 4:  $60,000 ($12.00/month, employee-paid supplemental coverage)

Life Reduction Benefit

Classes 1 & 4 only:  To 65% at age 65, and to 50% at age 70

Accidental Death and Dismemberment  

Class 1:  $50,000

Class 2:  $11,000

Class 3:  $5,500

Class 4:  $60,000 (Supplemental)

Accelerated Death Benefit If you become terminally ill, are less than age 60, and are not expected to live more than 12  months, you may request up to 50% of your life insurance amount up to $175,000.  The minimum Accelerated Death Benefit is $3,000.  A doctor must certify your condition in order to qualify for this benefit.  The death benefit will be reduced by any benefits paid under this option.  You should consult your tax and/or legal advisor before applying for this benefit.

*Life benefit amount for children under 6 months of age is $100.
