This link allows you to search for a specific doctor or view a list of Participating or Preferred Providers.  Follow the simple instructions in this site.  On the first field of the search, "Insurance Plan", choose "Preferred Provider Organization - PPO" for your PPO Plan.

Use this BlueCard® Doctor and Hospital Finder to find healthcare providers within the U.S., Puerto Rico and U.S. Virgin Islands. For providers outside of the U.S., please consult our list of BlueCard Worldwide® Hospitals.

Pharmacy - Blue Cross of Idaho

This link allows you to view Participating Pharmacies through WellPoint Pharmacy Management.  Click on Pharmacies, Pharmacy Finder, then Pharmacy Finder again, then enter your search criteria. 

DENTAL PROVIDERS:

Delta Dental

Delta Dental has Participating Providers that will not balance bill you beyond your deductible and coinsurance.  Make sure the Delta Premier box is checked in Section 1.

VISION PROVIDERS:

Vision Service Plan (VSP)

This link allows you to view Participating Vision Service Plan Providers.  You must have your Member ID number ready (usually your Social Security Number or Health Plan Subscriber Number), then enter your personal information and click "Submit".
