Eligibility
|
You
are generally eligible for Life insurance coverage if you
are an active, permanent full-time employee and work
at least 20 hours per week.
|

2: High School Athletic Directors - $50,000 Class
3: All other Eligible Employees - $25,000 |

Life
Reduction Benefit |
Life
insurance amounts will reduce to 65% at age 65, to 50%
at age 70, and terminate at retirement. |

Accidental
Death and Dismemberment
|
If
you sustain an accidental Bodily Injury which results
in loss of life, sight, or limb within 90 days of the
injury you are entitled to certain additional
benefits. |
|
Accelerated
Death Benefit |
If
you become terminally ill and are not expected to live
more than 12 months,
you may request up to 75% of your life insurance
amount to a maximum of $25,000.
A doctor must certify your condition in order
to qualify for this benefit.
The death benefit will be reduced by any
benefits paid under this option.

Conversion
Privilege |
If
your insurance terminates because your employment
terminates, or certain other reasons you may be
entitled to convert all or part of your Group Life
Insurance to an Individual Life Policy without
submitting evidence of insurability.

|
Waiver
of Premium
|
If Totally
Disabled prior to age 60, prior to termination of your
employment, disability has lasted for 6 months, and
proof of Total Disability is received within 12 months
after Premium payments end, your Life Insurance will
continue without Premium payments for 12 months. |

*An
unmarried Dependent child may be covered to age 25 if proof is
given to the employer that he is attending an accredited
college or university on a full-time basis.
